Education for Patient Empowerment
Education on diet and lifestyle, tailored to the patient, with set goals and targets for patient empowerment.
Patient Monitoring and Support
Dedicated regular phone calls and in-clinic visits for patient compliance.
Nurse Led Care
First point of call for patient for their HF. Escalates to cardiologist. Provides ongoing reports to GP / multidisciplinary team as needed.
